Acrobat 6 replaces wrong page

Without changing operating system (10.4), or version of Acrobat (6), we've had a few instances lately where we've replaced a page in a PDF document, and had the replacement page inserted elsewhere in the document in addition to where we wanted the page to go.

We can't recreate this, but like I said it's now happened on at least three different jobs.

Generally when we replace a page we have a long document that we are replacing individual pages on.

Has anyone else seen this?
