Default Add/Edit fields to PDF files

I am trying to edit/add to an Adobe Acrobat PDF Form and I need help! I have gone thru all your help menus and I am stuck! I have deleted the fields on the original PDF form and I am not able to undo to get them back. I need to add field names to corresponding boxes. Is there a way I can do this, without having to start the entire form over again? I figured if you can delete fields Acrobat can add fields? If someone could give me the instructions on how to add the fields that would be great!

If you have not saved the PDF after deleting the fields, close it without saving, and re-open it. The your form field changes are not fully "committed" unless you save the PDF.

Yes, adding new fields is a feature with Full Acrobat - in version 6 go to
Help > Advanced Editing > add form fields
    for the details.

We do not have the 6.0 version, we only have the Adobe Acrobat 5.0 version. Is there a similar way to add form fields in 5.0. Please reply as soon as you can.

    Thank You,

Use the form tool on the tool bar (F) and then draw a box where you want the form to appear. Then a menu pops up that allows you to select the form field parameters.
