Add Tags and Images disappear

When tagging a document to be accessible to the visually impaired (using Add Tags to Document), some of my images disappear. Poof. I can manually draw a box around the now blank space (using Touch Up Reading Order) and Acrobat will recognize it as an image that needs alternate text but the image is gone. I manually drew a box around our company logo and half of the logo disappeared.

This does not happen in all my pdfs that I try to tag (all have our logo and rarely does it disappear).

Mac OS 10.3.9, Acrobat Prof 7.0.8, pdf created from Quark 6.5

How are you creating the PDF - through Distille or PDF Export from Quark?

What is the object type that disappears?

What happens if you quit Acrobat and then view using RELOW mode under
the VIEW menu? Any difference with the TOUCH UP ORDER tool?
