Approval Proces
I come from a print background and am just entering interactive. Does anyone
know of a plug-in or a simple process to create a web-form to allow various
parties to approve and/or comment on a flash component.
What I'd love is a plug-in for flash that creates the HTML from flash's
publish command that will create a check-off area under the flash animation.
Other than that I'd love to hear what any of you have done to get the various
people to approve an animation (the idea of printing a form that needs to be
mailed or routed around seems counter intuitive to interactive media)

We have compiled a list of items that need to be approved and "sign-off" via
email. In our case, the client doesn't sign-off of technical criteria... that's
done in-house. We have a check list that the account managers refer to when
checking the work. We only get the client to sign off of the visual aspects,
flow and usability. So far email sign-offs have been sufficient CYA material.

I agree about the client sign-off. I am most interested in the workflow of the
internal approval. We have more than an account manager that needs to
sign-off, more of a QA from proof-reading & timing to Art direction and
styleguide conformity. Thank you very much for replying. If I find anything
I'll let you know if you are interested. We are thinking of developing an HTML
slug where we can place the animation.

I am currently trying to develop an approval system exactly like the one you are describing. I had envisioned the system to work as an index.php file that calls all .swf media content into a webpage with file information (dimensions, date uploaded, etc.) beneath the animation. There would also need to be a place for various approval sign-offs (I was thinking simple checkboxes that write data to a .txt file.) Did you end up successfully developing an approval system for flash content?
