Default Broken submission form...

Heya,
At www.reveration.org/email/devotional.htm I have a submission form
located on the left hand side. I took the code from a php file and typed it
into the table on my devotional.htm file, however, when you click the submit
button it brings up the error page. Can anyone help?
I think it could be because the code for the form itself was copied from
a PHP file, therefore in the html file it would be rendered inactive.

-Thanks
