The docs say nothing about where the inserted item would be placed, and I would only expect it to be placed as it's own item as you've found to be the case. As Massimo said, please file at the wish form what you would like to have happen.

Please elaborate on this.

One suggestion might be to create a Startup command that reads in this file and makes the necessary modification the first time DW is started after installing your extension.
