can't figure out how to do this form

I am trying to finsh up this e-commerce site and can't figure out how to make
the forms in order for the (size,style and quantity) of a shirt to be known
when the item is purchased.
I would really appreciate it if you could help me resolve this. the way I want

Hi E-commerce sites are not usually so simple since your database needs to
be connected to a very complex logic. To be able to process taxes,
users_id, user status, checkouts, sessions, discounts, payment gateways
etc.

For that reason we created MX Kommerce which is an entire solution on
e-store applications.

however if you already know this and just want an input for the specific
form generation process we have a tool called impakt which allows you to
easy deploy forms to register, update and create transactions with the
database.
