Crashing the server - Help!

My site has just started crashing the server. They said that every time my
site ran it crashed the server requiring it to be reset. Because this is
disrupting other accounts mine has been suspended.
Please help...
Below is the last log before the site crashed.

old versions of jet databases have been known to cause problems. did the
host give any more clues?
