Default Create a subfolder from a form

Hi, I want people to be able to tick a box on a form that will automatically
create a subfolder in a particular directory. The subfolder should pick up
it's name from a field in the form. This is so that users can then click on
a hyperlink to open that folder and store associated documents in it.

Any ideas how I can make this work.

    TIA
