Create Job Tickets via HTML browser page - Macromedia Dynamic HTML

Would it be possible to create a web page that would generate simple job tickets by using radio buttons to choose directions from sets of criteria. This would run from a server, inside my company only, and would not be on the web. The idea is that after clicking on a radio button to choose an item in each of several categories, then clicking a submit button, a page (PDF?) would be produced which would be the actual job ticket. It could then be printed. Can anyone advise if this is doable? Thanks!...

    This is not possible without a server side language such as php, asp, jsp etc.

    Choose a language of your choice and then learn out to use it to interact with
    the content of a submitted form.

    Are you using windows servers? Then use ASP and look into;

    Response.ContentType

    It works good with Excel and Word:
    Make the page to look like how you want the spreadsheet, then put this at the
    bottom after the content
    <% Response.ContentType = "application/vnd.ms-excel" %>

    Try this in a asp page:
