creating a PDF form someone can fill out andsend pdf back to me? tutorial?

Given the above file...I have Acrobat Pro...can I make it so people can fill out the fields and send the file back to me? If not...do I have to create a file from scratch. Is there a good tutorial for this?

If you have Acrobat 7 Pro, you can enable the typewriter tool for Reader 7.0.5 and later users who can then "type" on the form, save it, then return it to you.

To use the Typewriter tool, you'll need to be updated to Acrobat Pro 7.05 or later.
