Is there a way to create a questionnaire so that people can type in and answer the questions?
I would like to send out an email. I have created the form in Illustrator and then saved it as a PDF is there a way to convert this form so people can fill it out and then email it back to me?

I understand that you can if you have the new Acrobat Pro 8, but not with previous versions unless you are in a position to spend really considerable amounts of money.

Look at the Ted Padova blogs on the Acrobat User Community website about Acrobat 8 Professional:

In Acrobat 8 you can. There are a couple of steps involved. Make your PDF, or actually I guess with it being Illy you can open the .ai file in Acrobat. Then under the Forms menu choose Run Form Field Recognition. It does a really good job of it, but you may have to do some tweaking. Then under the advanced menu select Enable Usage Rights in Adobe Reader. Then your users with Reader 8 can fill in the forms and send them back to you.

    dear dori
when a form is created in the widows version (adobe designer) you get
the option to create a submit by email button to be added to the form
    ....
as a visible but non printing feature...
you tell the form what email addy things go to and the xml info does
    that ...

i have been wondering how to get my already created form to open on the
    mac...
because mac does not feature designer like the windows version does..
