Creating form

[Nickweaver]

Hi,
Am really stuck on creating this form using a dreamweaver 8...I have been
using dreamweaver version 4 for quite sometimes now I thought I should try
updating version 4 to version 8 and myself , aiming to be a professional web
developer in the near future. Could you just give me a clue or direct me to the
site where you think I can get much help...just creating form using dreamweaver
version 8.

What is it you are stuck on? Creating a form in either DW4 or DW8 is
essentially identical.
