Creating Forms w/DWMX & sending to database - Macromedia Dreamweaver

Hey there all...

Not sure where to begin, so here it goes. I have DWMX and have been tasked with creating a form which will upload via ASP to our SQL server.

This form should be able to have the ability to have attachments added to it (ex: .html, .doc, .ppt, etc...). Using the standard text boxes, drop-down menus and the like, once the information has been added to the form, it gets uploaded as one piece (attachment(s) and all) to our database (not via email).

Can someone point me in the right direction as where to start, and helpful extensions or tutorials, etc..? Are there any good books out there that might be able to help me out?

Sorry for being so vague, just not sure where to go with this, you know?

On the applications tag you will see an Insert Record option which will
create the form and the code for you to do the basic update.

The tricky part will the filename, especially if you want it to upload it to
the server. For that you will need something like the Pure ASP Upload from
