I have a Web host that:

    a) with their old WinNT plans one must call in and ask them to do it

    b) with their new Win2K+ plans they offer a user control panel.

(You do not install in on your machine; it's either in the plan or it
    isn't.)

It's whoever has control of the physical system. If your provider
gives you that control then they don't have to be involved. If you're
not leasing a server or virtual server then you likely don't have the
    control.

    Depends on provider and plan.

You don't. It's on the web server, and done by your provider. Or not
    done, it's your provider's choice.

Problem solved, I've changed the server path where web host server located
and create a different folder to place the DB.

    Thanks
