Email address in a form

    Default Email address in a form

    I have a form where people enter their e-mail address into a required field.
    What i am trying to do is when the user clicks submit and it sends the form to
    the designated email that the email address that was entered in the form field
    is put as the sender. Is this possible and how do I get it to do that?

    I am using a mailerdamon that is built into netsol's hosting package.

    Read their instructions for how to use that script. Surely it has a way to
    do this.

    I am looking for somthing similar.

    I am trying to create a "time off request form" for employees. When the
    employee enters our intranet, they would be able to complete the form to
    request time off. I would like the form to be sent first to their supervisor
    for approval. Once the supervisor approves it, I would like the supervisor to
    send it to me to file and process. I can either receive it as an attachment or
    as part of the body of the e-mail. As long as I have something in writing, I
    have no objection to either way.

    I tried doing it with Acrobat (I created a form) but users will need to have
    Acrobat Writer in order to be able to complete it and send it. Since not all
    employees have the acrobat application, I decided to create an HTML form. But
    employees will be required to enter the e-mail for their supervisor in order to
    get it approved (I don't want the form to come to me until approved by the
    supervisor.

    Any ideas how can I do this?:confused;
