Emailing "customized" survey forms

Here's what I want to do:
Send an email containing a link to a "customized" survey. By customized, I
mean some of the recipient's information is already included in the survey.
This information is provided in a form that is filled out at our company. When
the Submit button is clicked, the email is sent with the link to the survey.

What is happening:
The above scenario works fine if only one survey is sent out and responded to
- the customized information is correct for the recipient. However, if more
than one survey is sent out, the customized information is correct for the last
recipient, but all the other surveys also contain the same information as the
last recipient.

It's not clear from your code how the link to the customized survey is created.
I assume the form that is submitted adds a record to the database, and then
this record ID is the key to the survey? Sounds like either the record is not
being added or you are not getting the right ID when you email the survey.
