If you use the Distribute Forms wizard in Acrobat 8 Professional, it will create a data set file to receive the data from forms. That data set file has the capability to export the data as a CSV (comma-delimited) file.
InDesign CS2's Data Merge feature can import CSV files. However, you may want to use Excel to clean up the CSV file that gets created. You could use it to reformat numbers, remove empty fields, etc.
