Use the Forms Tools, on the Advanced Editing toolbar. There is
information in the Help file, but you have to carefully ignore what it
says about "Designer", a Windows only method of making a different
    kind of form.

Be sure you have at the START of a project a knowledge of the
limitations of forms. For example, users with the free Reader will not
be able to save or e-mail these forms.

There's plenty. In fact, that's how I taught myself how to use the tools.

Sure, go to Tools>Advanced editing>Show Forms Tool Bar (available in Acrobat Pro only)

You might also consider getting a 3rd party book on Acrobat which gives further explanation on the form tools. One good choice would be Ted Padova's Adobe Acrobat 7 PDF Bible.
