Thanks, we did use Javascript to highlight each field. Our form is here: <http://www.innovativesigns.com/InnovativeLRF106.pdf>
Each field highlights in gray when active, then goes transparent when not active.
If you ever find a solution to the non-blinking cursor problem, please let me know.

Mystery Solved ! (however, it cost me $39.95 at Adobe tech support)
Problem:
Certain text box fields do not display a blinking cursor when the user tabs to them or clicks on them.
Reason:
If a text box is not tall enough to accommodate the height of a blinking cursor, the cursor will not display at all.
Two solutions:
1 – Make the text box bigger.
2 - If you cannot make it bigger without ruining your design, and you are setting a specific point size in this field's properties, change that point size to AUTO and Acrobat will scale the blinking cursor down to fit within the field (as well as scale the user's entry to fit the field).
So simple, yet so frustrating when you have no clue what is causing the problem.
