I understand your view from the design phase but my experience is that the overwhelming majority of designers are too damn lazy to break out the components into separate live files. But I should qualify my viewpoint by stating I worked prepress prior to ascent of PDFs. I believe you can export a multipage doc to separate PDFs but not sure if each page will crop correctly when exported. Never had need to do it.

Martin sounds like he's developed a good relationship with his service providers. That's the real secret. I'd rather get crappy files from a polite, respectful customer who tries to understand the production cycle than perfect files from a spoiled crybaby who makes incessant, impossible demands. I can help the first customer build his or her skills and knowledge. The second customer, unfortunately, is beyond my ability to help.
Addendum after re-reading my post:
I don't mean to insinuate that Martin's files are crappy. He seems very knowledgeable and competent.

Well I seem to have started a lively discussion...
I guess what we are asking Adobe for is a system that allows multiple page sizes with a large paste-board that has a prepare for repro button making it repro house friendly.
I do agree that a lot of designers I have come across are lazy, don't understand computers, software nor printing. I guess Adobe can do little about bad designers though.

sizes in one FH document. My workaround is to export all pages to
individual EPS files including bleed area, run them through Distiller
and combine them into separate, multi-page PDFs of pages with identical
dimensions to keep things tidy.

I think that they have given us pretty much what you have asked for, but it just works differently than what you are used to.
As said in a previous post, the "Book Pallet pretty much does everything you have spoken of. Personally I like it better! I have been using Freehand for about 8 years and have spent the last 2 years converting all documents to Illustrator for obvious reasons.
Unfortunately, I think this is just the beginning of Freehand users having to look at different ways of adjusting there workflow to fit their need using Adobe products. The only thing I personally want from Freehand is find and replace, no arguments from me on that topic.
With the book pallet you can have multiple page sizes, convert all pages to PDF with crops. You can also sync ...Object styles, TOC Styles, Character Styles, Paragraph styles, Trap Presets and Swatches giving you a shared swatch pallet. You can even preflight and collect.
So Adobe is giving us an application that will handle multiple page sizes in one file.
The file would be named something like Davidsbook.indb

This is not correct. Transporting the package requires the .indb file AND the .indd files. With the book you may have all the features that you have mentioned, but it is by no means a single file.

It is true...I was referring to handle or by using a different word...manage.
No reference to packaging or transporting in that sentence.
To transport as one file convert it to .pdf
