Just to be sure, have you confirmed that the "Create PDF Bookmarks" option is turned on in the Table of Contents dialog box?

in the export dialog, you have to check "include bookmarks" AND "include hyperlinks" . . . even though you may not have any hyperlinks. the bookmarks in the PDF will then function properly. go figure.

I just did a quick test using 4.0.3 to make sure that the basic functionality works. It does. This suggests the following:
The update to 4.0.3 has somehow changed your settings and you're not noticing that the necessary checkboxes to activate bookmarks are now unchecked. There are two: first, in the TOC dialog, you have to check the box to make the bookmarks, then, in the Export PDF dialog, you have to check the box to say Include bookmarks.
If you're doing both of these and yet it is not working, then try trashing your preferences.
