Try deleting the prefs file by holding ctrl+alt+shift when starting pshop. If that doesn't work, uninstall and reinstall. If that doesn't work, um. . .

the first one and the second one doesnwt work
are you SURE you deleted the prefs? did you get a dialog asking you if it was ok to reset your photoshop settings? if not you were too slow and missed it. it took me about 5 tries to catch it right. See this faq topic:
