Export to HTML is at the best flaky. It's like taking a tangerine and
converting it to an orange - they're similar but not quite the same.
HTML export has been replaced in InDesign CS with Package for GoLive, which
(I guess, I don't have GoLive) gives you a stack of data that GoLive is able
to work with to produce HTML. Doing it this way suggests that Adobe has
decided that to do the job properly you need an export filter the size of an
independent programme, and that is a bit much to expect of a filter.

If you are a design masochist, export a PDF from IDCS and then open the PDF in Acrobat 6 Pro. From Acrobat 6 Pro, export HTML.
The Package for GoLive feature (afaik) was designed because screen design is not print design. There are other things to consider in making a web layout as opposed to a print layout. Trying to re-work the exported HTML from ID2 was a nightmare. IDCS' export of the package to GoLive makes web design much more manageable.
