Why would you not want the reader to read "bullet" every time it encounters a bullet? How else will readers know it is a bulleted list? IMHO... -

If you have Acrobat 8, you can scan a paper form, have the form
auto-recogonized, and then touch-up the results. But to be honest, it
is not all that hard to create a form manually.
