I think it's algorithm is:
if there is an image on the clipboard
image size = big enough for it
else
image size is the last value you set it to
resolution = last value you set it to

What Colin said but just to add to his second option; it will only duplicate the image size/resolution of the last SAVED document.
EDIT: Actually maybe it depends on your operating system because I'm sure at work where I have W 2000 it defaults to the last SAVED doc image size (regardless of cropping/canvas size change) but I just tested it now at home on W XP and it changed to the canvas size of the last opened document without saving!!!
Can the operating system control that??? Actually I'm not so sure now...I'd like to withdraw my comment. Can everyone please visualise me sheepishly backing away in shame!!! Thank you.

I think you were on the track. I don't think PS remembers the size of an image you opened. I think you need to either save or explicitly set the size of a new doc.
