Not the first time I've responded to my own post...

As I re-read the post (a neurotic habit) I noticed that I have an older version
of the Sonnet upgrade card extension. I will try again with the latest and
    re-post if the problem changes.

The actual question is still: what is different if you restart with the shift
key down and if you just turn off all Extensions and Control Panels? Why
    is there a difference?

Holding down SHIFT at startupdisables all Extensions, all Control Panels, all
"faceless background applications" (many printer drivers are actually FBAs, for
example), and everything in the Servers and Startup Items folders.
