    Default Online Survey

    I am trying to create an online survey and I am wondering what the best way to
    do this is.

    It will have about 30 questions sent out to 12,000 people and 5 questions
    would change each month.

    I have a survey now that is a bit smaller than that it really isn't dynamic at
    all, so that the process is quite convoluted. I want this to be easy to process
    and submit variables to and from the database.

    Does anyone have any ideas, technologies, or frameworks that will work well in
    doing this?

    Actually sending the survey to people would be difficult. I would send them a
    link to the survey. I make the questions static as they dont change much, but
    have been thinking about loading the questions in a db. Then of course I store
    all the results in a table and then create the reports. I dont see a right or
    wrong way in doing this, as long as you follow the rules of programming.
