Once it's transparent, whatever is behind it should show through. At
99.9% it will simply overprint what's behind it.

A 99.9% opacity will only show .1% more of the background through. Opacity only shows the background through the image because it made the inverse opacity % of the image transparent.
It does not cause the image to overprint. The image will still knock out the background.

Overprinting Images
Use Direct Selection Tool to select the image, then apply Overprint Fill in Appearance Window.
