What outputs to PDF if you use the following PDF Export settings from
    InDesign CS3?

    1. PDF/X-1a:2001
    2. Smallest file size

    ??

Also - try turning ON the overprint settings from
    InDesign / View /
Acrobat / Advanced / Print Production

Let us know if there's any difference?

I had a similar problem with discoloration and resolved it by saving the InDesign file as a JPG then creating a PDF of the JPG through Acrobat.

I've seen the "negative effect" thing, but it's only been when viewing a PDF through the Mac OSX Mail program. Once the PDF was downloaded and opened in Acrobat they were correct.

Are your client's looking at these PDFs with Preview or with Acrobat? Do they have an old version of Acrobat?
