Click on the guide and then use the control panel (CS) or transform palette to position it by typing in the coordinate you want.

Hi Lars--
Select the guide, then enter a value in the X or Y field in the Control palette or Transform palette. These values won't be in pixels, but they will be in your current measurement units.

That's not all--because the guides are objects, you can copy and paste them in position on any page; i.e., guides copied from the 2-3 spread will paste into the exact same position on any other spreads in the file. In structured documents, I define all my guides on my master pages, so they automatically appear on the live pages to which I apply that master. I also put all my guides on a separate layer atop all other layers in the file. That way I can toggle them on and off while still leaving the margin guides showing. It also allows me to lock the layer so I don't accidentally move or delete a guide.
Finally, here's a trick you can't do with Freehand: drag guides onto the pasteboard for bleed guides.
