Screen tint color issue
There were two different covers done that had a problem. The first was simply a matter of highlighting the title on the cover and changing the color to a new PMS color. It changed the color on screen and in the small boxes above the color swatch box, but no color showed up in the color list to indicate the color that had been applied to the title. When the book was printed the press couldn't identify the color used and printed it in white.
The second job involved changing several areas of screened (tinted) color at the same time by changing the 70% tint to a new color. All the areas changed color on screen once again and all but one of the items did show a linked color in the color/swatch box. The single item then printed white since the machinery couldn't identify the color used on that item.

Re: Screen tint color issue
I've had similar situations in the past when a color is created in the
mixer, or by using the eyedropper from a raster image, and applied to an
object, then the color is subsequently added to the swatch list. Even
though the color is the same, the pre-swatch-list instance of the color
will not count as an instance of the swatch color so be sure to add the
color to the swatch list before applying it to any objects.
If you already have documents exhibiting this problem, using the 'Name All
Colors'
Xtra (Xtras Menu>Colors>Name All Colors) will integrate the orphaned color
instances to the color swatch.
