simple calendar to enter date in forms

I have put together an asp form linking to an acess database, where a user can
enter the working hours of each of their team. I would like the form to have a
drop down calendar so that the date format remains consistant.

Is there a simple way of doing this?

Check ou the following expert calendar extension:

The cost is $25, but it is a great extension and works very well. The support
from the website is excellent if you have any trouble using the extension.
