Submitted drafts do not appear

I'm trying to configure a new Web site to work with Contribute 4. We
want to use the draft approval process which we have not attempted with
Contribute 2 in the past. However, I'm having the following problem:

1. A user in the "writer" role creates or edits a page.

2. The "writer" then sends the page for review.

3. The sent page appears in the writer's "Drafts I'm editing" list in
the Drafts console. It never goes to the "Sent Drafts" list.

4. The draft never shows up in the administrator's draft console, nor
does the administrator receive an email notification.

5. If the writer goes back and tries to edit the file from the "Drafts
I'm editing" list, a pop-up appears with the following message: "The
administrator has sent this draft to another user. Your changes to the
draft were lost."

Problems in step four occur regardless of whether the writer chooses
"all administrators in the 'administrative' role", or one specific
administrator.

The setup is a Dreamweaver-managed site on a Windows/Apache server.
Contribute connections are done through Secure FTP. The writer has
Windows, SFTP and Contribute permissions to edit/create files in the
relevant folder.

Oddly enough, this worked at first, but then stopped working.

I am having *exactly* the same problem, exactly as you describe it here - did you ever get to the bottom of this? Any reply from Adobe?
