Vicky, I'm having _exactly_ the same problem you are describing, and it's quite icomprehensible to me. Not that that is any comfort to you! But I wonder if this is a bug - somehow. I'm now using the new 3.01 version, but that didn't improve on this erratic behavior which I first experiences in the 3.0. Because of a deadline, I had to _manually_ copy/paste-edit TOC items for a publication - how humiliating!

Aha! On reading this, I got a little flash. So I re-arranged my headings only on the left-right axis - and my TOC became correct or wrong according to the left-right placement!!! The left-right placement overrules the top-bottom placement, instead of giving _priority_ to the top-bottom axis, irrespective of the right-left axis. Just try this yourself.
ADOBE: This is clearly a BUG. Please fix ASAP. But it'll probably be in the 3.02 patch, released half a year from now - so we are doomed to manually editing your TOCs forever. Jeez, how I pray for more _frequent_ patches (and I'm not even a praying man).
Addendum: I didn't read the in-between messages, stupid me - and I only now see that other people have already discussed this "known issue." Fine! That increases perhaps our chances of getting a bug-fix before Halloween?

It seems that we will often have to resort to a kludge fix: put copies of our headings on an invisible layer also, and arrange the necessary left-right order there. It will work, but it is damn kludgy.

OK Bob Index seems so difficult and TOC is easy, I will try Index instead.
