Uploading a local .CSV file into a MySQL Database - MySQL

Hello All, I need to create a form that will upload a .CSV file from my local computer to a MySQL database. The form will include the file, along with some additional manual categories that need to be entered into a table. I have searched the net on this and haven't been able to find anything yet. I will be using PHP. Any help, tutorials, anything would be greatly appreciated. Thanks, -Nate...

    Hello All,

    I need to create a form that will upload a .CSV file from my local
    computer to a MySQL database.

    yet. I will be using PHP. Any help, tutorials, anything would be
    greatly appreciated.

    In user comments to that page, a third good way (LOAD DATA INFILE) is
    suggested by Alexander Skakunov:
