I have to change the setting for each file
What Setting are you referring to?
Custom setting is remembered for each file but when I open a new image,
the setting goes back to the default

Re: when saving slices how you set "setting" the way you like?
When you do "save for web" and "save" and window pops up "save optimized as" and in the third drop down there is an option for "settings" where you can specify "custom" "background.."defaul" or "other"
and then I usually choose other... and then on the second dropdown in the setting window, I go to "saving files" where you specify file naming etc...
