White box around psd files with transparency

Hello people, I work in the image department for a local newspaper, now for the gossip section there is heavy use of cut out images with text running around them. For that we've always used psd files, the process of saving them is opening, cutting out the subject putting them into cmyk mode and saving them as psd files which has always worked, but every now and then a psd file will show up correctly in indesign, everything looking as it should but after printing instead of transparency there's a white box around the image.

I'm thinking that it might have something to do with the pdf process, but this is a very random occurance.

Any help would be appreciated.
