WSE - Username Token

Hello:

I am trying to build a webservice and client that will pass a username
and password in the SOAP header.

However, when trying to call the AddUserToken() method, I get an
exception:
Microsoft.Web.Services.Security.SecurityFault:
The security token could not be authenticated or authorized.

Any ideas why this would be happenning? Any help would be greatly
appreciated!
